Full-Time Licensed Practical Nurse

Pathways Community Living is seeking a Full-Time Licensed Practical Nurse to provide compassionate care to residents with intellectual and developmental disabilities (IDD) and dementia in our residential community located in Allison Park, PA 15101. You will work onsite as part of a dedicated team, delivering individualized nursing care while fostering meaningful connections with residents and their families.

Responsibilities

• Conduct comprehensive assessments to monitor health changes and identify care needs
• Implement individualized care plans tailored to each resident's requirements
• Assist residents with daily living activities while maintaining dignity and respect
• Collaborate with a multidisciplinary team to coordinate comprehensive care
• Communicate effectively with families, addressing concerns and providing progress updates
• Document all nursing assessments, interventions, and care activities
• Adhere to safety protocols and maintain a supportive environment
• Provide specialized dementia care with attention to behavioral and cognitive changes

Required Qualifications

• Active Pennsylvania Licensed Practical Nurse (LPN) license
• Strong interpersonal and communication skills
• Ability to work effectively with residents, families, and interdisciplinary team members
• Critical thinking skills to assess situations and make informed care decisions
• Excellent observational skills to detect subtle changes in residents' conditions
• Adaptability and resilience to respond to dynamic care environments
