Optum Home & Community Care is seeking a full-time Nurse Practitioner to conduct in-home health assessments for Medicare Advantage and other plan members across Avoyelles, Evangeline, Rapides, and Allen Parishes in Louisiana. This is a non-prescribing, field-based role focused on improving health outcomes through comprehensive assessments, education, gap closure, and collaboration with primary care providers.

Clinical Setting

You will work in patients' homes, conducting preventive care assessments that help members stay well and navigate the health care system. This is part of an interdisciplinary care environment where you will connect patients to key support services and address their physical, mental, and social needs.

Responsibilities

  • Conduct comprehensive in-home assessments including past medical history review, medication reconciliation, vital signs, and physical examination
  • Perform evidence-based screenings and point-of-care testing as appropriate
  • Identify and document diagnoses for care management and treatment planning
  • Communicate assessment findings to members' primary care providers to address gaps in care
  • Recognize urgent and emergent situations and intervene appropriately
  • Educate members on disease processes, medications, and medication compliance
  • Address social determinants of health and provide referrals as needed

Required Qualifications

  • Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • Active, unrestricted Louisiana Nurse Practitioner license without current disciplinary actions or practice limitations (or ability to obtain by start date)
  • Active and unrestricted driver's license
  • Reliable transportation to complete home visits
  • Ability to navigate varied home environments and perform physical assessments (ability to sit, stand, and kneel as needed)
  • If practicing in Louisiana under independent practice authority, you must obtain approval from your licensing board if required. New hires who are eligible but have not applied must do so within one month of starting. If not eligible at hire, you must begin working toward eligibility within one month and apply for approval within three months of becoming eligible.

Schedule

This position offers flexible scheduling with the ability to flex your time between 8 a.m. and 8 p.m. over a seven-day work week. No on-call, weekend, or holiday requirements.

Compensation and Benefits

Compensation for this specialty generally ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonus and is based on several factors including local labor markets, education, work experience, and may increase over time based on productivity and performance in the role.

A $10,000 sign-on bonus is available for external candidates. Additional compensation may be available for qualified candidates who are fluent in languages other than English and use those language skills during member visits.

Additional Requirements

Candidates are required to pass a drug test before beginning employment.
