Full-time Home Health Licensed Practical Nurse- King County

Signature Healthcare at Home is seeking a Full-time Licensed Practical Nurse to provide direct patient care in King County, Washington. In this role, you will work under the supervision of a registered nurse to implement individualized plans of care, manage patient treatments and medications, and support patients in maintaining health, activity, and independence in their home settings.

Clinical Setting

You will deliver home health services across King County as part of Signature Healthcare at Home, a leading home health provider in Oregon and Washington. The organization is backed by The Pennant Group network, which operates 300+ locations nationwide.

Responsibilities

• Provide direct patient care as defined in the State Nurse Practice Act
• Implement current nursing practice following comprehensive patient assessment and individualized plan of care
• Document patient services accurately and timely to reflect the plan of care
• Evaluate patient and family/caregiver education needs and provide information pertinent to diagnosis and care plan
• Use and prepare equipment and supplies effectively and efficiently
• Provide appropriate pain and symptom management; evaluate patient response to treatments and medications
• Report significant changes in patient condition to the patient's RN Case Manager
• Participate in quality assessment, performance improvement activities, and professional development
• Complete mandatory annual education requirements to maintain competence
• Meet productivity expectations
• Perform other duties as assigned by the Registered Nurse or Clinical Manager

Schedule

• Monday through Friday, 8:00 AM to 5:00 PM
• Full-time position

Compensation

• $37 to $42 per hour, depending on experience

Required Qualifications

• Graduate of an accredited practical nurse or vocational nursing program
• Currently licensed as an LPN/LVN in Washington State
• Valid driver's license with a personally owned, insured automobile in good working order
• Current CPR certification
• Ability to perform physical demands of the role: prolonged or considerable walking or standing; lifting, positioning, and transferring patients; lifting supplies and equipment; reaching, stooping, bending, kneeling, and crouching; visual acuity and hearing sufficient to perform required nursing skills

Preferred Qualifications

• One to two years of nursing experience
• Community health or medical/surgical nursing experience
• Knowledge of geriatric population and their common needs and concerns
