Full-time Home Health Licensed Practical Nurse- King County

Signature Healthcare at Home is seeking a Full-time Licensed Practical Nurse to provide direct patient care in the King County area. You will work under the supervision of a registered nurse, implementing individualized plans of care and helping patients achieve and sustain health, activity, and independence in their chosen home settings.

Clinical Setting

This is a home health nursing position serving patients throughout King County, Washington. You will visit patients in their homes to deliver skilled nursing care.

Responsibilities

• Provide direct patient care as defined in the State Nurse Practice Act
• Implement current nursing practice following comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services
• Evaluate need for and provide patient and family/caregiver education pertinent to diagnosis and individualized plan of care
• Use and prepare equipment and supplies effectively and efficiently
• Provide appropriate pain and symptom management, and evaluate patient response to treatments and medications
• Report significant changes in patient condition to the patient's RN Case Manager
• Participate in quality assessment, performance improvement activities, and professional development
• Complete mandatory education requirements annually to maintain competence
• Meet productivity expectations
• Perform other duties as assigned by the Registered Nurse or Clinical Manager

Required Qualifications

• Graduate of an accredited practical nurse or vocational nursing program
• Currently licensed as an LPN/LVN in Washington State
• Valid driver's license with a personally owned, insured automobile in good working order
• Current CPR certification
• Ability to perform physical demands of the role: prolonged or considerable walking or standing, lifting and positioning patients, lifting supplies and equipment, reaching, stooping, bending, kneeling, and crouching
• Adequate visual acuity and hearing to perform required nursing skills

Preferred Qualifications

• One to two years of nursing experience
• Community health or medical/surgical nursing experience
• Knowledge of geriatric population and their common needs and concerns

Schedule

• Full-time position
• Monday through Friday, 8:00 AM to 5:00 PM

Compensation

• $37 to $42 per hour, depending on experience
