Job Description

Pine Park Health is seeking a full-time Nurse Practitioner to provide primary care to older adults living in senior living communities across the San Jose and Santa Clara area. This field-based role brings healthcare directly to patients' homes, offering prevention, screening, chronic condition management, lab work, and diagnostic testing in a value-based care model designed to keep seniors healthy and independent.

You will manage a smaller panel of complex patients, develop personalized care plans, and work closely with RNs, MAs, and care navigators to deliver coordinated, proactive care. This role emphasizes meaningful patient relationships and time to practice medicine well, with less time spent navigating traditional clinic models.

Clinical Setting

You will provide care onsite at partner senior living communities across the South Bay, traveling between locations as needed. All providers work from home on Fridays, creating dedicated time for documentation, care coordination, and professional development.

Responsibilities

  • Provide comprehensive primary care to older adults in their senior living communities, building ongoing relationships with patients and their care teams
  • Develop and manage individualized care plans that reflect each patient's medical needs, goals, and preferences
  • Deliver monthly Chronic Care Management (CCM) visits and complete associated documentation
  • Collaborate with RNs, MAs, care navigators, and community staff to coordinate care and support continuity between visits
  • Lead regular clinical huddles to review patient needs, coordinate follow-up, and support effective team-based care
  • Partner with community leadership and care staff to implement care plans in each patient's living environment
  • Collaborate with the Medical Director to improve clinical workflows, care standards, and policies, including responses to emerging diseases

Required Qualifications

  • Active, unrestricted California Nurse Practitioner license with Furnishing Number
  • Active, unrestricted DEA registration (Schedules II–V)
  • Interest in caring for medically complex older adults and delivering coordinated, preventive primary care
  • Willingness to provide onsite care across the South Bay, including driving between partner communities
  • Adaptability and comfort working in a growing, evolving care model
  • Strong collaboration skills and ability to value input from every member of the clinical team
  • Clear, effective communication with patients, families, and clinical team members
  • Comfort with tech-enabled care tools

Preferred Qualifications

  • Geriatrics experience
  • AGNP-C certification
  • Value-based care experience
  • Home-based, house-call, or mobile practice experience

Schedule

This is a full-time position with flexibility around patient and community needs. You will work onsite at partner communities with mileage reimbursement for work-related travel. All providers work from home on Fridays.

Compensation and Benefits

  • Total target cash: $152,000–$195,000 (base plus bonus), based on experience and qualifications
  • Equity
  • Work from home Fridays for all providers
  • 6+ weeks of annual PTO, including vacation, sick time, and paid holidays
  • Medical, dental, and vision coverage for you and your dependents
  • 8 weeks of paid parental leave
  • Flexible spending accounts for health and dependent care
  • 401(k)
  • Monthly wellness allowance
  • Annual professional and personal development stipend
  • Company-paid UpToDate subscription
  • Company-issued cell phone
  • Mileage reimbursement

Final compensation reflects NP experience, training, licensure, and geographic market.
