We are seeking a Family Nurse Practitioner (FNP) to join an outpatient primary care and urgent care clinic in Brooklyn, NY. This role offers the opportunity to deliver comprehensive patient care in a fast-paced, high-volume clinical environment while working with a collaborative multidisciplinary team.

Clinical Setting

You will work in an outpatient clinic providing primary care and urgent care services. The setting is fast-paced and high-volume, requiring strong clinical judgment and the ability to work independently while collaborating with a care team.

Responsibilities

  • Deliver comprehensive primary care in an outpatient setting
  • Diagnose, treat, and manage acute and chronic conditions
  • Perform physical exams, order labs, and interpret diagnostic results
  • Develop and manage individualized patient care plans
  • Educate patients and families on prevention, treatment, and long-term care
  • Maintain accurate, high-quality EMR documentation
  • Collaborate with a multidisciplinary care team
  • Contribute to patient growth through outreach and engagement initiatives
  • Support clinic operations, workflow efficiency, and scheduling

Required Qualifications

  • Active New York FNP license (APN certification required)
  • BLS/ACLS certification
  • Experience in outpatient primary care or urgent care
  • Strong clinical judgment and ability to work independently
  • Excellent communication and patient engagement skills
  • Ability to work in a fast-paced, high-volume environment

Schedule
