Advantmed is seeking a licensed Nurse Practitioner to conduct structured, in-home wellness and risk-adjustment assessments for diverse patient populations. This is a 1099 independent contractor PRN role offering flexible weekday scheduling with no evenings, weekends, or on-call duties. You will work in the Plummer, Idaho area with occasional support needed in Pinehurst, Idaho (approximately once per month).
This role is designed for NPs seeking reliable, ongoing supplemental income with operational structure and support. It requires consistency and reliability, with a minimum commitment of approximately 30 hours per month on an ongoing basis. This is not intended for casual or very short-term engagement.
Responsibilities
- Conduct in-home Annual Wellness Visits and chronic condition assessments
- Perform focused histories, exams, vitals, and medication reviews
- Perform screenings including Hemoglobin A1C, Spirometry, KED (Kidney Health Evaluation), DRE (Diabetic Retinal Exam), and FIT
- Educate members on preventive care and chronic disease management
- Document visits in the EMR to support value-based care and risk adjustment
- Work independently and professionally in the field
Schedule
- Flexible weekdays only; no evenings, weekends, or on-call
- Choose your own weekday availability
- Most providers complete 4-5 visits per day based on availability
- Visits are ready to schedule immediately
Compensation
- Approximately $110 per completed assessment
- Paid mileage
Travel Requirements
- Daily travel within a 55-mile radius of Plummer, Idaho
- Willingness to support Pinehurst, Idaho as needed with anticipated visit frequency of approximately once per month
- Access to reliable transportation to travel to members' homes within the designated area
Required Qualifications
- Active, unencumbered Nurse Practitioner license in Idaho
- 3 or more years of patient care experience preferred
- Comfortable with EMR systems and independent work
- Ability to work independently
- Reliable transportation
