Molina Healthcare is seeking a Field Nurse Practitioner to provide screening, preventive primary care, and medical care services to members in non-clinical settings where they feel most comfortable. You will work primarily in-home, in community settings, nursing facilities, and pop-up clinics, striving to ensure member progress toward desired health outcomes while contributing to quality and cost-effective care delivery.

Clinical Setting

You will provide care across diverse settings including members' homes, skilled nursing facilities, community locations, and pop-up clinics. This role requires significant travel within your assigned region, with regular driving up to 120 miles per day and occasional extended mileage requiring overnight hotel stays.

Responsibilities

  • Provide general medical care and care coordination to various patient populations including adults, women's health, pediatric, and geriatric members
  • Perform comprehensive evaluations including history and physical exams to identify gaps in care and conduct preventive assessments
  • Address both chronic and acute primary care complaints and assess medical urgency appropriately
  • Establish and document reasonable medical diagnoses
  • Seek specialty consultation as appropriate
  • Order and perform pertinent diagnostic laboratory and radiology testing, using diagnostic tests judiciously within limited resource environments
  • Recognize when a member's needs exceed your scope of knowledge and escalate to physician oversight as needed
  • Create and implement medical plans of care
  • Schedule appointments for visits when appropriate
  • Provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization
  • Perform face-to-face in-person visits in various settings
  • Conduct face-to-face visits via alternative modalities based on business need, leadership direction, and state regulations
  • Order bulk laboratory orders targeting specific member populations
  • Perform alternating on-call coverage to triage urgent lab results and pharmacy inquiries and develop appropriate plans of care
  • Participate in community-based pop-up clinics to build relationships and address healthcare gaps
  • Prescribe medications and perform procedures as appropriate
  • Perform timely medical records documentation in the electronic medical record (EMR) system
  • Collaborate with fellow nurse practitioners to develop best practices
  • Actively participate in regional meetings
  • Obtain and maintain cross-state licensure in other states as needed based on business requirements
  • On occasion, walk flights of stairs while carrying up to 50 pounds of equipment
  • Engage in practices constituting the practice of medicine in collaboration with and under the medical direction and supervision of a licensed physician to the degree required by state laws

Required Qualifications

  • At least 1 year of experience as a nurse practitioner or equivalent combination of relevant education and experience
  • Active and unrestricted national certification from the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
  • Current state-issued license to practice as a Family Nurse Practitioner (FNP), active and unrestricted in state of practice
  • Prescriber Drug Enforcement Agency (DEA) license with authority to prescribe per state qualifications, active and unrestricted in state of practice
  • Current Basic Life Support (BLS) certification
  • Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job-related travel requirements
  • Ability to work within a variety of settings and adjust style as needed when working with diverse populations, various personalities, and personal situations
  • Ability to work independently with minimal supervision and demonstrate self-motivation
  • Responsive communication in all forms
  • Ability to remain calm in high-pressure situations
  • Ability to develop and maintain professional relationships

Travel Requirements

This position requires regular local travel based on state and contractual requirements. You will drive up to 120 miles per day on a regular basis to various locations within your assigned region. Extended mileage beyond 120 miles may be required on special project days, which may include overnight hotel stays.
