Field Base, Nurse Practitioner or Physician Assistant - Portland, OR

SCAN is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide and coordinate primary care to SCAN members in the Portland, Oregon area. This is a field-based position where you will deliver care to an assigned panel of patients primarily in senior living communities and skilled nursing facilities, with a focus on keeping older adults healthy and independent.

Clinical Setting

You will work in community-based settings, primarily senior living communities and skilled nursing facilities, providing in-home care to SCAN members. This is a patient-facing role with significant travel requirements throughout the Portland area.

Responsibilities

• Provide primary care to an assigned panel of members in their place of residence, including physical and wellness exams, assessments, urgent medical visits, and telephonic triage
• Administer vaccines and order and interpret diagnostic tests
• Initiate goals of care conversations with patients and their families
• Manage chronic diseases such as dementia, diabetes, heart failure, and COPD
• Identify patient healthcare needs and develop comprehensive care plans in collaboration with families and living community staff
• Independently manage your visitation schedule in line with established protocols, adjusting based on member needs and acuity
• Collaborate with case management teams to ensure timely access to necessary services
• Work collaboratively with Primary Care Physicians for clinical oversight and guidance
• Connect with specialists, hospitals, ancillary care providers, and community-based organizations to ensure continuity of care
• Monitor and address clinical quality gaps in partnership with other care providers
• Document member visits and interactions in a timely manner according to established protocols
• Serve as an ongoing direct resource for members, families, and community support staff
• Participate in virtual health visits to promote accurate triaging and reduce drive time
• Participate in the on-call program available to members after hours and on weekends
• Participate with the Integrated Care Sales team in community events to build member awareness
• Maintain professional knowledge through educational workshops and professional publications
• Contribute to quality healthcare goals by closing care gaps, attending high-risk group discussions, and optimizing clinical workflows

Required Qualifications

• Master's degree in Nurse Practitioner or Master's degree from an accredited PA program (education in Adult/Geriatric medical care preferred)
• Oregon Nurse Practitioner license OR Oregon Physician Assistant Certification (PA-C) in good standing
• Knowledge of complex chronic disease treatment, including dementia, diabetes, heart failure, and COPD
• Ability to identify healthcare needs and refer and coordinate with other resources and programs
• Knowledge of HCC and ICD-10 coding and documentation
• Knowledge of wound care and management
• Knowledge of CMS guidelines and Medicare Advantage Managed Care
• Comfort with initiating goals of care and end-of-life discussions
• Strong interpersonal skills and change agent mindset to build relationships with healthcare teams and patients
• Knowledge of and ability to maintain HIPAA requirements
• Adaptability and openness to change and problem-solving
• Basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and openness to new technology
• Valid driver's license, automobile insurance, and reliable transportation
• Ability to stoop, kneel, and crouch; ability to bend, reach, lift, pull, push, and carry up to 10 pounds regularly and up to 50 pounds occasionally

Preferred Qualifications

• Wound Care Certificate
• Palliative Care Certificate
• Multilingual and multicultural background

Travel Requirements

• Travel 75% or more of the time in the assigned Portland area
• Must have a valid driver's license, automobile insurance, and reliable personal transportation

Schedule

• Participation in on-call program available to members after hours and on weekends
• Remote/field work mode

Compensation and Benefits

• Base salary range: $125,400 to $179,300 annually
