Family Nurse Practitioner

Tuolumne Me-Wuk Indian Health Center is seeking a Family Nurse Practitioner to provide comprehensive primary care and perioperative services to our community. As a licensed independent practitioner, you will assess and diagnose patients, prescribe medications and treatments, and coordinate care across our clinic and community health services. This is a full-time, exempt position reporting to the Medical Director with a Monday through Friday, 8am to 5pm schedule.

Clinical Setting

You will work in a community health center located in California's Sierra Nevada foothills, serving the Tuolumne Me-Wuk Indian Health Center patient population. The center is committed to providing culturally sensitive, high-quality care to diverse tribal communities.

Responsibilities

• Independently perform comprehensive history and physical examinations for perioperative and primary care patients, assessing both normal and abnormal findings
• Evaluate and treat acute injuries and illnesses
• Provide preventive healthcare services and monitor the effectiveness of interventions
• Order and interpret diagnostic and therapeutic tests
• Prescribe medications and implement treatment plans in compliance with standardized procedures, federal and state regulations, and Indian Health Service guidelines
• Coordinate follow-up services including lab work, imaging, specialist referrals, and appointment scheduling
• Manage admissions, transfers, and discharges while collaborating with a multidisciplinary team
• Consult with the Medical Director or Provider Practice Lead when cases exceed your professional scope
• Provide patient and family education on health promotion, risk management, and disease prevention
• Interface with community organizations to identify and connect individuals with necessary resources
• Act as a resource for staff development in your area of expertise
• Engage in continuing education and participate in quality assurance initiatives
• Ensure patient records adhere to QA guidelines
• Update Medi-Cal forms and clinic protocols as needed
• Assist with inventory management
• Participate in the Quality Assurance Committee
• Report public health concerns such as epidemic outbreaks, poisonings, major accidents, or other critical incidents to the Department of Health Services, local health departments, and fire authorities within 24 hours via telephone, followed by written confirmation

Required Qualifications

• Bachelor of Science in Nursing (BSN)
• Master's or Doctorate in Nursing from an accredited program
• Active and unrestricted Nurse Practitioner (NP) license in California
• DEA registration
• Prescriptive authority in California
• Cultural sensitivity and ability to interact respectfully and effectively with diverse tribal communities
• Successful completion of pre-employment TB test, background check, and drug screening

Preferred Qualifications

• Two or more years of experience working with Native American populations in a healthcare setting

Physical Requirements

This position requires the ability to walk, stand, and sit for prolonged periods; climb stairs and step-stools; stoop, kneel, crouch, balance, and reach; push, pull, and lift up to 30 pounds; raise objects overhead; operate computers and phones with manual dexterity; hear and speak clearly with good verbal and written communication skills; perform repetitive motions of the wrists, hands, and fingers; work in outside environmental conditions of heat and cold; walk on uneven ground; and safely operate and drive a motor vehicle.
