Family Nurse Practitioner (NP)

Liberty Resources is seeking a Family Nurse Practitioner to join the Integrated Health Care Center in Fulton, NY. In this role, you will provide comprehensive primary care to patients of all ages using a team-based, patient-centered approach. You will work collaboratively with the Medical Director and mental health staff to deliver integrated healthcare services to individuals with diverse physical, emotional, and social needs.

Responsibilities

• Perform patient interviews, health histories, physical examinations, mental status assessments, and diagnostic tests to determine diagnosis and current health status
• Provide medical crisis intervention and counseling through direct visitation or telephone contact
• Complete evaluations, medication reviews, care plans, and progress reports
• Manage clients' medication regimens, prescribe drugs and medication treatments as indicated by diagnosis and lab results
• Provide primary care, care coordination, and medication education to clients and families
• Participate in treatment planning and crisis intervention services when necessary
• Provide health education to clients and families to support positive health outcomes
• Work collaboratively with mental health staff to provide integrated healthcare

Required Qualifications

• Current Certificate and registration to practice as a Nurse Practitioner in New York State
• At least one year of experience providing direct services in a family medicine setting
• Eligibility for full and unconditional participation in the Medicaid and Medicare programs
• Ability to effectively use an EMR and complete all required documentation in a timely, professional manner
• Flexibility and orientation to family-centered work
• Cultural sensitivity and ability to incorporate cultural understanding into service provision
• Ability to regularly sit, stand, walk, bend, climb stairs, and lift objects up to 10 pounds as required

Preferred Qualifications

• Experience with individuals diagnosed with mental illness or comparable experience

Compensation

• Pay range: $120,000 to $145,000
