Jane Pauley Community Health Center, a Federally Qualified Health Center (FQHC) serving Indiana, is seeking a Family Nurse Practitioner to provide comprehensive primary care across the lifespan. This role offers the opportunity to deliver full-scope clinical services to diverse and underserved populations while working within a collaborative, team-based model that integrates behavioral health and support services.
Clinical Setting
Jane Pauley Community Health Center provides integrated, whole-person care to individuals and families across Indiana regardless of income or insurance status. You will work as part of an interdisciplinary team focused on clinical excellence, community impact, and health equity.
Responsibilities
- Conduct patient assessments, physical exams, and diagnostic evaluations
- Diagnose and manage acute and chronic conditions across the lifespan
- Develop and implement individualized treatment plans
- Prescribe medications and perform procedures within scope of practice
- Provide preventive care, screenings, and health maintenance services
- Educate patients and families on disease management, prevention, and wellness
- Collaborate with care teams to ensure coordinated, patient-centered care
- Support transitions of care and appropriate referrals within and outside the network
- Participate in quality improvement initiatives and care gap closure efforts
- Support population health strategies and value-based care programs
- Monitor patient outcomes and contribute to continuous improvement efforts
- Maintain accurate and timely documentation within the Electronic Health Record (EHR)
- Ensure compliance with HIPAA, regulatory, and organizational standards
- Support coding, documentation accuracy, and billing requirements
Required Qualifications
- Master's or Doctoral degree in Nursing from an accredited program
- Current, unrestricted Indiana Nurse Practitioner license
- Board certification as a Family Nurse Practitioner (FNP)
- Prescriptive authority and DEA licensure (or ability to obtain)
- Strong clinical assessment, diagnostic, and decision-making skills
- Excellent communication and collaboration skills
Preferred Qualifications
- Experience in primary care, community health, or FQHC settings
- Experience working with diverse or underserved populations
- Familiarity with EHR systems such as Epic
- Experience in team-based or integrated care environments
