Family Nurse Practitioner (FNP)

PathCare Health is seeking a full-time Family Nurse Practitioner (FNP) to join our team at our Brooklyn, NY office. This is a fully in-person position where you will evaluate and treat patients of all ages, managing a steady patient schedule in a fast-paced clinical environment.

Responsibilities

• Evaluate and treat patients of all ages
• Perform patient assessments and physical examinations
• Diagnose and manage acute and chronic conditions
• Review medications and medical history
• Develop appropriate treatment plans and referrals
• Complete and lock documentation accurately and promptly
• Work closely with clinical and administrative team members

Required Qualifications

• Active New York State Nurse Practitioner license
• Family Nurse Practitioner (FNP) certification
• Active NPI
• Enrollment with New York State Medicaid and eligibility to bill Medicaid
• Strong clinical and documentation skills
• Ability to work reliably in a fast-paced environment
