Family Nurse Practitioner- Direct Primary Care

Southern Oregon Wellness is seeking a Family Nurse Practitioner (FNP) to join our direct primary care practice in Southern Oregon. This role offers the opportunity to practice relationship-based, patient-centered healthcare with longer appointment times and a manageable patient panel, allowing you to focus on prevention, wellness, and developing meaningful long-term relationships with patients rather than high-volume visits.

Clinical Setting

Southern Oregon Wellness operates as a direct primary care practice designed to provide accessible, relationship-based healthcare. The practice model emphasizes spending quality time with patients, addressing underlying health factors, and creating individualized care plans with an evidence-based approach.

Responsibilities

• Provide comprehensive primary care to patients across the lifespan
• Diagnose and manage acute and chronic medical conditions
• Perform preventive health evaluations and wellness visits
• Develop individualized treatment and health-maintenance plans
• Provide patient education, counseling, and lifestyle guidance
• Build lasting relationships with patients and families
• Order and interpret appropriate laboratory and diagnostic studies
• Coordinate specialty care when needed
• Participate in improving the patient experience and practice processes
• Practice with an emphasis on prevention, whole-person care, and evidence-based medicine

Required Qualifications

• Current Family Nurse Practitioner (FNP) certification
• Active Oregon NP license with appropriate prescribing privileges
• Graduate of an accredited nurse practitioner program
• Current national board certification
• Strong clinical assessment and decision-making skills
• Excellent communication and interpersonal skills
• Ability to work independently while collaborating with the broader care team

Preferred Qualifications

• Strong primary care training and interest in direct primary care
• New graduates with strong primary care training are encouraged to apply
