The Family Nurse Practitioner delivers comprehensive health care in accordance with the philosophy, standards, and policies of The Institute for Family Health. The Nurse Practitioner is responsible for diagnosis and treatment of common and chronic illnesses, with a focus on health promotion and disease prevention activities including patient education and counseling.
Responsibilities
Clinical Care
- Conduct health assessment interviews and perform complete physical assessments
- Order and interpret diagnostic tests
- Develop individualized plans of care based on diagnosis and client needs
- Prescribe medications and treatments as appropriate
- Record, report, and interpret client responses to the plan of care
- Make home and hospital visits to clients and take calls as appropriate
- Provide medical phone coverage as directed by supervisor
Patient Education and Community Engagement
- Teach, counsel, and assist individuals and families to assume responsibility for illness prevention and health promotion, maintenance, and restoration
- Identify available community resources and assist clients in utilizing them
- Collaborate with community agencies to provide care to clients and groups in the community as approved
Team Collaboration
- Work collaboratively with the health care team and make referrals to other disciplines as indicated
- Collaborate with other team members in providing regular health screening and health promotion measures
- Practice as a member of the interdisciplinary care team via effective and respectful communication
- Provide feedback to the Nurse Manager, Practice Administrator, and/or Medical Director regarding policies and practices
Age-Specific Care
- Identify physical, behavioral, and emotional characteristics typical for assigned age groups
- Modify approaches based on patient age-specific needs and responses to treatment
- Use age-appropriate communication techniques

- Participate in staff development and serve as a preceptor for selected students and staff
- Maintain collaborative practice agreement with physician as required by New York State law
- Attend relevant workshops, conferences, and seminars
- Provide presentations and educational offerings
- Mentor and develop staff
- Serve as a role model and resource person for professional and paraprofessional staff
Education Requirements
- Bachelor's degree in nursing
- Master's degree in nursing
- Graduate of an accredited Nurse Practitioner program
Licenses and Certifications Required
- Current New York State Registered Nurse License and Registration
- Nurse Practitioner Certification and Registration
- Current Practice Agreement
- Certified or eligible for certification by recognized specialty organization
- Current BCLS certification
- Current Infection Control certification
- Current Child Abuse certification

- Strong Family Nurse Practitioner clinical experience
