Aspire Indiana Health is seeking a Nurse Practitioner for Family Medicine and Primary Care to provide integrated primary health care services and medication management to children, adolescents, and adults in an outpatient setting. This position serves as an independent and interdependent member of the medical services team, delivering integrated general medical and behavioral health care. You will work extensively with adults diagnosed with substance use disorders and/or serious mental illness within residential programs and housing through the Federally Qualified Health Center (FQHC).
Responsibilities
- Provide quality integrated primary health care services to patients across all ages
- Manage medications for children, adolescents, and adults
- Perform as an independent and interdependent member of the medical services team
- Deliver integrated general medical and behavioral health care
- Work with adults diagnosed with substance use disorders and/or serious mental illness in residential programs and housing settings
Required Qualifications
- Master's degree in Nursing or Doctorate degree in Nursing from a college or university accredited by the US Department of Education
- Recognition as a Registered Nurse (RN) with Prescriptive Authority by the State of Indiana
- Current Drug Enforcement Administration (DEA) certificate
- Current State of Indiana Controlled Substance Registration (CSR)
- ANCC or other certification as a Family Nurse Practitioner
- Current BLS certification
Preferred Qualifications
- One year of supervised experience in primary care with Severely Mentally Ill (SMI) and/or substance use disorder populations
Additional Requirements
- Drug screen, TB test, and extensive background checks required, including Criminal History, Sex Offender Registry Search, State Central Registry Check, Education Verification, and Professional References
- Candidates are strongly encouraged to have a flu shot and be fully vaccinated against COVID-19 prior to joining Aspire
- Adherence to CDC protocols including wearing masks, social distancing, and sanitizing
