Evening Shift Registered Nurse (RN) or Licensed Practical Nurse (LPN)

Wabasso Rehabilitation and Wellness Center is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join the overnight shift team. In this role, you will provide high-quality, patient-centered care to residents in a skilled nursing facility setting, working closely with an interdisciplinary team to develop and implement individualized care plans.

Responsibilities

• Provide direct patient care, including administration of medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans in collaboration with the multidisciplinary team
• Perform routine nursing tasks such as vital sign monitoring, bathing, and dressing
• Maintain accurate and up-to-date records of patient care and treatment plans
• Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
• Provide emotional support and education to residents and their families
• Participate in quality improvement initiatives and maintain a safe and clean environment
