Evening Shift Licensed Practical Nurse (LPN)

Warren Nursing & Rehab is seeking an Evening Shift Licensed Practical Nurse (LPN) to deliver patient care and support to residents in our skilled nursing and rehabilitation facility in Warren, Ohio. In this role, you will work under the supervision of experienced nursing staff to provide high-quality care to our patient population.

Responsibilities

• Provide high-quality patient care and support under the supervision of nursing staff
• Administer medications and perform wound care
• Assist residents with daily living activities as needed
• Develop and implement individualized care plans to meet the unique needs of residents
• Collaborate with the interdisciplinary team to ensure seamless transitions and optimal outcomes
• Maintain accurate and detailed records of patient care and treatment
• Participate in ongoing education and training to stay current with industry standards and best practices

Required Qualifications

• Current LPN licensure in the State of Ohio
• High school diploma or equivalent
• Ability to work effectively in a fast-paced environment with minimal supervision
• Excellent communication and interpersonal skills
• Flexibility to work varied shifts, including evenings and weekends

Preferred Qualifications

• Associate's degree in nursing or related field
