Evening Shift Licensed Practical Nurse (LPN)

Caledonia Senior Living & Memory Care in North Riverside, Illinois is seeking a Licensed Practical Nurse (LPN) for the evening shift. In this role, you will provide direct care to residents in a senior living and memory care setting, working as part of a collaborative interdisciplinary team to deliver high-quality, individualized care.

Responsibilities

• Provide direct care to residents, including administering medications, wound care, and personal care
• Develop and implement individualized care plans in collaboration with the interdisciplinary care team
• Monitor and report changes in residents' condition to the nursing team and other appropriate staff members
• Collaborate with other departments, such as social services and activities, to ensure comprehensive care for residents
• Participate in quality improvement initiatives and maintain accurate and up-to-date records

Required Qualifications

• Current LPN license in the state of Illinois
• Basic Life Support (BLS) certification
• Ability to work effectively as part of a team and communicate effectively with residents, families, and other staff members
• Strong observational and assessment skills, with the ability to identify changes in residents' condition
• Ability to lift, transfer, and move residents with assistance, as needed
• Basic computer skills and familiarity with electronic medical records
