Interim HealthCare is seeking a compassionate and experienced Hospice Registered Nurse (RN) to deliver exceptional, patient-centered end-of-life care in the Greenville, SC area. In this vital role, you will provide comfort, understanding, and support to patients and their families during one of life's most meaningful transitions.
Schedule
- Full-time position
- 4:30 PM to 8:30 AM, Monday through Thursday
- Mileage reimbursement provided
Responsibilities
- Complete initial and comprehensive assessments of patients and families to determine hospice needs
- Conduct ongoing assessments of patients' physical status and psycho-social status of patients and families
- Initiate and develop plans of care in collaboration with the interdisciplinary team (IDG), patients, families, and physicians
- Revise IDG plans of care as patients' and families' status and needs change
- Coordinate care among IDG members and external providers to ensure continuous assessment of patient and family needs and implementation of the IDG plan of care
- Evaluate and coordinate services provided to patients and families in accordance with the IDG plan of care
- Administer medications and treatments as prescribed by physicians in accordance with nursing standards of practice
- Provide education and interventions to support effective management of patients' pain and other physical symptoms
- Assess patients' and families' responses to interventions and treatments
Required Qualifications
- Graduate of an accredited Registered Nurse (RN) program
- Active South Carolina RN license or Compact License
- Minimum one year of nursing experience
