Copy of Registered Nurse-Home Health Visits

BAYADA Home Health Care is hiring Registered Nurses to provide home health visits throughout Monroe County, Pennsylvania. This is a PRN, part-time, or full-time position offering flexible schedules, one-on-one patient care, and strong work-life balance.

Clinical Setting

You will make home visits to clients in designated geographic territories throughout Monroe County, providing direct patient care in patients' homes.

Responsibilities

• Follow designated care plans in accordance with patient needs
• Make home visits to clients in assigned geographic territories
• Administer medications, perform wound care, and carry out treatments and procedures as assigned
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Provide case management and care coordination
• Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet

Required Qualifications

• Current active Registered Nurse license in Pennsylvania
• Minimum of one year of recent, verifiable clinical (medical/surgical) experience
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills; prior experience with electronic medical records (EMR) preferred
• Ability to travel to cases as assigned

Preferred Qualifications

• Prior home care experience
• Experience with electronic medical records (EMR)

Travel Requirements

You must be able to travel to cases as assigned throughout your designated geographic territory.

Compensation

Base pay ranges from $52.00 to $64.00 per hour, depending upon qualifications and subject to negotiation.
