We are seeking a Licensed Practical Nurse to provide direct patient care in a skilled nursing or long-term care setting. In this role, you will work with residents across various care needs, administering medications and treatments, monitoring health status, and assisting with daily living activities. You will collaborate with the nursing team, supervise support staff, and maintain accurate medical records while ensuring a safe and comfortable environment for residents.
Responsibilities
- Administer medications and treatments as prescribed by physicians via oral, intravenous, subcutaneous, and intramuscular routes
- Monitor and record vital signs, observe residents for changes in condition, and report significant findings to the supervising RN or physician
- Provide wound care including dressing changes and manage treatments such as IV therapy, catheter care, tracheostomy care, and ostomy care
- Assist residents with activities of daily living such as bathing, dressing, grooming, mobility, and feeding
- Collect specimens for laboratory testing and perform basic diagnostic procedures
- Maintain accurate, timely documentation in residents' medical records in compliance with facility policies and regulatory requirements
- Supervise and delegate tasks to Certified Nursing Assistants and other support staff as appropriate
- Provide emotional support to residents and families, and communicate effectively about care plans and health status
- Ensure a safe, clean, and comfortable environment while adhering to infection control protocols
Required Qualifications
- Current New York State LPN License in good standing
- Current Basic Life Support (BLS) / CPR certification
- Strong clinical skills in medication administration, wound care, and patient assessment
- Excellent communication, interpersonal, and organizational skills
- Ability to work effectively as part of a team and supervise unlicensed personnel
- Must pass a background check, drug screening, and meet all New York state and facility health requirements including vaccinations and TB test
Preferred Qualifications
- Minimum 1 year of nursing experience, ideally in skilled nursing, long-term care, subacute rehab, or geriatric care
