Join Valley Health System as a Community Health Nurse providing skilled nursing care to patients in their homes. In this full-time day position, you will coordinate personalized care plans, deliver clinical services including wound care, drain management, catheter changes, and medication management, and collaborate with multidisciplinary teams to support patient wellness and independence.
Responsibilities
- Assess patients' health needs and develop individualized care plans
- Provide skilled nursing care including wound care, drain management, catheter changes, and medication management
- Conduct case conferences with multidisciplinary teams including therapists, home health aides, dietitians, social workers, physicians, and other healthcare professionals
- Monitor patient progress and coordinate services
- Educate patients and families about medications, treatments, and home safety
- Connect individuals with community resources
- Maintain accurate documentation
- Ensure safe and effective care delivery
Clinical Setting
Home-based care through the Home Care Team 4 at Kraft Center.
Required Qualifications
- Graduate from an accredited School of Nursing
- Current and valid New Jersey RN license or legal permission to work from the New Jersey Board of Nursing
- Current and valid CPR certification
- Valid driver's license, vehicle registration, inspection sticker, and appropriate insurance coverage including collision
- Computer and keyboard skills
- Medical terminology knowledge
- Effective oral, written, and presentation communication skills
- Ability to work accurately with attention to detail while managing multiple tasks
- Ability to function competently in stressful situations and changing work environments
- Ability to work cooperatively with patients, families, and multidisciplinary team members
- Effective time management and ability to respond quickly to emergencies
- Critical thinking and clinical reasoning skills for problem-solving and care delivery
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)

Driving Requirements
A current and valid driver's license, vehicle registration, inspection sticker, and appropriate insurance coverage are required. You must maintain an acceptable driving record for the duration of your employment as long as driving remains a job responsibility. All employees must adhere to HR Policy 108 procedural guidelines.
Compensation and Benefits
Pay Range: $99,424.00 - $124,280.00 annually (full-time equivalent). Actual salary varies based on role, experience, education, and licensure.
