CNA | Certified Nursing Assistant | Community Aide

Midland Care Connection, a not-for-profit community-based organization, is seeking a Certified Nursing Assistant (Community Aide) to provide essential hands-on care to clients in their homes. This position helps older adults and seriously ill individuals remain safe, healthy, and independent in their familiar home environment while receiving support for activities of daily living and basic household needs.

Clinical Setting

You will work in clients' homes across multiple counties in northeast Kansas, supporting individuals enrolled in Midland Care Connection's full continuum of care services, including hospice, palliative care, home health, and the PACE (Program of All-Inclusive Care for the Elderly) program.

Responsibilities

• Assist clients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility
• Help with basic household tasks, meal preparation, and medication reminders
• Monitor and report changes in a client's condition to nurses or supervisors
• Support client comfort and safety
• Offer companionship and emotional support
• Work independently in clients' homes while following established care plans
• Observe, document, and report client information to nursing staff or supervisors

Required Qualifications

• Current Certified Nursing Assistant (CNA) certification in the state of Kansas
• High school diploma or GED
• Ability to provide hands-on assistance with activities of daily living, including bathing, dressing, grooming, toileting, and mobility
• Basic knowledge of infection control, safety practices, and patient rights
• Ability to observe, document, and report changes in a client's condition
• Strong communication and interpersonal skills
• Ability to work independently in clients' homes while following care plans
• Physical ability to perform job duties, including lifting, bending, and standing for extended periods
• Valid driver's license, reliable transportation, and willingness to travel to clients' homes
• Ability to pass background checks, health screenings, and required trainings

Preferred Qualifications

• Previous experience working with older adults, individuals with chronic illness, or those with mobility or cognitive impairments
• Prior home health, hospice, palliative care, or PACE program experience
• Experience assisting with light housekeeping, meal preparation, and medication reminders
• Familiarity with interdisciplinary care teams and community-based healthcare models
• CPR/First Aid certification
• Demonstrated ability to provide emotional support and companionship to clients

Schedule

• Full-time position
• Monday through Friday, 8:00 AM to 5:00 PM

Shift Differentials

• Weekdays 5:00 PM to 8:00 PM: $1.00 per hour
• Weekends 8:00 AM to 5:00 PM: $1.50 per hour
• Weekends 5:00 PM to 8:00 AM: $2.00 per hour

Compensation and Bonuses

• CNA: $18.15 per hour or higher
• HHA (Health Home Aide): $20.50 per hour or higher
• CNA Bonus: $1,000 paid in two increments at 6 months and 1 year of employment
• HHA Bonus: $1,500 paid in two increments at 6 months and 1 year of employment
• CNAs receive paid training to obtain their HHA license after 60 days of employment

Travel Requirements

You must have a valid driver's license, reliable transportation, and be willing to travel to clients' homes throughout the service area.
