Clinical Manager-Registered Nurse Home Health Immediate Opening

Granite Peak Home Health is seeking a Clinical Manager (RN) to oversee all home health patient care within a designated region. This leadership role combines clinical expertise with operational management, directing assessment, care coordination, and staff supervision to ensure high-quality, patient-centered home health services.

Responsibilities

• Provide oversight of all patient care services and personnel, directing clinical activities
• Provide direct oversight of assessment, planning, implementation, and evaluation of patient and caregiver care
• Lead, manage, and hold clinical staff accountable for compliance, quality, and timely care delivery
• Schedule and conduct weekly interdisciplinary team (IDT) meetings and routine leadership meetings in alignment with CMS and PDGM guidelines
• Develop and implement processes, policies, and procedures for patient care services
• Assist in preparing and administering the agency budget
• Monitor census trends and adjust staffing, revenue, and expenses accordingly
• Ensure adherence to company policies, employee handbook, and regulatory standards
• Participate in hiring, evaluating, and terminating personnel
• Oversee onboarding, in-service training, and continuing education programs
• Conduct quarterly check-ins and annual performance reviews
• Ensure accurate patient documentation and reporting
• Maintain adequate clinical supplies and equipment
• Support quality assurance and patient safety initiatives
• Assist in developing organizational and clinical goals
• Ensure compliance with all local, state, and federal home health regulations
• Stay current on changes in home health and nursing practices, including CMS, third-party payor, and state licensure requirements

Required Qualifications

• Bachelor's Degree in Nursing
• Current Montana RN License
• Demonstrated proficiency in OASIS-E assessment
• Ability to exercise initiative and independent judgment
• Proficiency with electronic health records (WellSky) and web-based systems
• Must pass background check and not be listed on the OIG exclusions list
• Must have a cellular phone for work-related use
• Valid driver's license, reliable transportation, and motor vehicle insurance

Preferred Qualifications

• Minimum of 2 years of nursing experience in Home Health
• Management experience
• Knowledge of Montana Home Health (DPHHS), CMS guidelines, PDGM, and reimbursement programs
