Certified Nursing Assistant-Transitional Living-Full-Time

Brooks Rehabilitation is seeking a Certified Nursing Assistant to provide direct patient care in our skilled nursing facility at University Crossing in Jacksonville, Florida. This full-time position focuses on caring for patients with brain and spinal cord injuries in a transitional living environment.

Clinical Setting

You will work in a skilled nursing facility specializing in rehabilitation care for patients with neurological injuries. Brooks Rehabilitation is recognized among the top 20 rehabilitation systems nationally by U.S. News & World Report and operates as a comprehensive rehabilitation system with inpatient hospitals, outpatient services, skilled nursing, assisted living, and memory care.

Responsibilities

• Answer patient call lights and respond to patient needs
• Provide direct activities of daily living (ADL) care including bathing, showering, dressing, and feeding for dependent patients; assist partially independent patients
• Toilet patients or provide bedpan-urinal assistance; clean patients after incontinence episodes
• Serve patient trays and feed dependent patients; monitor for choking and aspiration risk
• Provide oral hygiene and grooming assistance
• Turn and reposition patients to prevent pressure injuries
• Make occupied and unoccupied beds
• Assist with washing patient personal clothing as needed
• Take and record vital signs (temperature, pulse, respiration, blood pressure) and report abnormalities to the nurse
• Empty catheter bags and report intake and output to the nurse
• Provide condom catheter care
• Obtain specimens for laboratory tests
• Perform bladder scans
• Weigh patients and record measurements
• Transport patients to therapies, appointments, and other departments; transport equipment and specimens as required
• Clean and maintain patient rooms, bathrooms, unit kitchen, utility rooms, and common areas
• Monitor patients for safety to prevent falls, injuries, and wandering
• Interact with patients and families; provide emotional support
• Record patient care information including vital signs, intake and output, and weight
• Maintain confidentiality of patient information and comply with patient rights policies
• Maintain a clean and safe environment; identify and report hazards
• Adhere to universal precautions and infection control policies
• Complete annual equipment and skills competency checklists
• Participate in performance improvement activities
• Comply with attendance, dress code, and conduct policies
• Communicate appropriately with coworkers and provide courteous service to patients, visitors, and physicians

Required Qualifications

• Current Certified Nursing Assistant license from the State of Florida
• Basic Life Support (BLS) certification
• Current BLS card (hands-on)

Schedule

• Full-time position: 32 hours per week
• Available shift: 7:00 AM to 3:00 PM
• Location: 6210 Beach Blvd, Jacksonville, FL 32216
