The opportunity

Job Description

Summit Ridge Skilled Nursing is seeking a Certified Nursing Assistant to provide direct patient care in our 29,454-square-foot skilled nursing facility located in Douglas, Wyoming. This is a non-exempt, hourly position reporting to the Director of Nursing, Unit Manager, CNA Coordinator, Resident Care Coordinator, or Director of Clinical Services.

Summit Ridge is a Medicare and Medicaid certified facility specializing in Alzheimer's and dementia care, with a dedicated 20-bed Special Care Unit. We also offer full in-house physical, occupational, and speech therapy services. Our integrated team of RNs, LPNs, and CNAs provides 24-hour care in a warm, home-like setting with private and semi-private rooms, a bariatric suite, and modern amenities.

Responsibilities

  • Provide supportive services to nursing staff, medication technicians, and other team members as assigned
  • Assist residents with activities of daily living in accordance with individualized care plans and facility policies
  • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts
  • Coordinate dining room services at assigned mealtimes, including setup, cleanup, meal tray delivery, feeding assistance, and documentation of meal intake
  • Deliver nutritional supplements to patients at assigned times and assist with intake as necessary, documenting accordingly
  • Assist with weighing patients according to facility policy and record weights in designated locations; report weight changes to nursing staff and supervisors
  • Monitor and track resident skin condition, reporting any pressure areas, skin breakdown, or skin tears to nursing staff and supervisors
  • Assist nursing staff with toileting program activities
  • Complete daily flow sheets documenting that assigned tasks were completed and progress of each resident as directed by supervisor
  • Keep nursing staff and supervisors informed of factors that interfere with assigned work, such as resident refusal or reassignment to another unit
  • Report changes in resident condition to the resident's nurse and supervisor
  • Treat all patients with dignity and respect while promoting and protecting patient rights
  • Follow all facility policies, procedures, and state and federal regulations
  • Maintain confidentiality of protected health information in all verbal, written, and electronic communications
  • Report any noncompliance with policies, procedures, or regulations to appropriate personnel
  • Report any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of patients to supervisor and/or administrator
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with facility policy
  • Participate in all life safety and emergency drills and trainings; fulfill assigned responsibilities during emergency plan activation
  • Report work-related injuries and illnesses immediately to supervisor
  • Follow established infection control policies and procedures
  • Complete all assigned training and skills competency assessments as a condition of employment
  • Attend care plan meetings for assigned patients
  • Participate in quality assurance and facility assessment activities as needed
  • Accept CNA assignments as staffing needs require
  • Use proper lifting and body mechanics while delivering patient care

Required Qualifications

  • High school diploma or equivalent
  • Current CPR/BLS certification

  • No disciplinary action in effect against certification

Preferred Qualifications

  • Certified Nursing Assistant in good standing with the state
  • At least one year of experience as a certified nursing assistant in a skilled nursing facility or assisted living facility

Physical Requirements and Working Conditions

  • Ability to read, write, speak, and understand the English language
  • Ability to move intermittently throughout the workday
  • Ability to walk, stand, sit, bend, and stoop for extended periods
  • Ability to bend, stoop, kneel, crouch, and perform overhead lifting and other common physical movements as needed
  • Ability to lift, move, push, or pull a minimum of 35 pounds
  • Dexterity of hands and fingers to perform resident care
  • Ability to assist in evacuation of patients during emergency situations
  • Meets general health requirements according to facility policy, including medical and physical exams and verification of immunity status to various infectious diseases
  • Ability to work beyond normal working hours and on weekends and holidays when necessary
  • May be subject to falls, burns from equipment, and odors throughout the day
  • May encounter reactions from dust, tobacco smoke, disinfectants, and other air contaminants
  • Subject to exposure to infectious waste, diseases, and conditions including AIDS, Coronavirus, Hepatitis B, and Tuberculosis
  • May be subject to hostile or emotional patients, family members, visitors, or personnel
  • Must not pose a threat to the health and safety of other individuals in the workplace

Compliance will be a factor in evaluating job performance. Violations, including failure to report violations, will result in disciplinary action, up to and including termination.
