Milestone, Inc. is seeking a Certified Nursing Assistant to provide direct care and support to individuals with developmental disabilities. Under the supervision of a Team Leader, you will deliver personal care services, implement treatment and rehabilitation programs, and participate in activity therapy and recreational activities.
Responsibilities
- Provide personal care services and assist with activities of daily living, including cleanliness, personal hygiene, table manners, and social skills development
- Implement Individual Service Plans and carry out daily lesson plans and active treatment programs as directed by supervisors
- Monitor individuals' whereabouts continuously and maintain accurate, up-to-date records of all individual programs and behavior changes
- Accurately report, record, and document specific occurrences including seizures, toileting, intake and output, menses, weight, injuries, and medical emergencies; contact nursing personnel and supervisors promptly when appropriate
- Monitor all individuals' health needs and contact supervising personnel and nurses in a timely manner
- Motivate and encourage individuals to promote their development and treatment; observe and report behavior and reactions
- Communicate shift information clearly in the communication log and verbal report to incoming staff, including specific problems related to behavior, health, or building maintenance
- Assist individuals with safe transportation to and from recreational and day program activities when authorized to drive Milestone vehicles
- Provide appropriate supervision and assistance to individuals while in the community, helping them participate in outings to their fullest abilities
- Ensure individuals' safety during emergency situations and demonstrate knowledge of proper disaster procedures, fire extinguishers, alarm systems, and emergency exits
- Maintain the physical plant in a clean, safe, and pleasant condition; perform spot cleaning as needed and report maintenance concerns
- Work cooperatively with laundry and housekeeping staff to maintain individuals' clothing and household linens in clean, well-fitting, and safe condition
- Use all mechanical lifts, belts, chairs, wheelchairs, and other appliances appropriately and clean them as necessary
- Demonstrate knowledge of individual diets and adhere to prescribed meal plans
- Participate in team meetings twice per month and cooperate with scheduling to ensure adequate facility coverage
- Maintain confidentiality concerning individuals, peers, and the agency
- Comply with all Milestone, Inc. policies and procedures, including those regarding human rights and mandatory reporting to appropriate agencies (OIG, DCFS, IDPH) when necessary
- Demonstrate reliable, prompt attendance and flexibility in working with team members to provide adequate facility coverage
- Perform other duties as directed or assigned to provide appropriate care
Required Qualifications
- Must be at least 18 years of age
- High school diploma or GED
- Must pass the TABE test
- Must complete habilitation aide certification
- Must be able to intervene physically in behavioral and health-related crisis situations with individuals
Preferred Qualifications
- Nurse's aide experience
- Personal care experience
