Bronte Health and Rehab Center, a skilled nursing facility, is seeking a Certified Nursing Assistant for day shift. In this role, you will provide essential support to nursing staff and other healthcare professionals while delivering direct care to residents. You will work as part of an interdisciplinary care team to help residents maintain their quality of life and independence.
Responsibilities
- Provide certified nursing assistant services to assigned residents according to care plans, facility policies, and supervisor direction
- Assist residents with activities of daily living including bathing, grooming, dressing, and toileting
- Assist with lifting, turning, moving, positioning, and transferring residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts
- Coordinate dining room services at mealtimes, including setup and cleanup, meal tray delivery, feeding assistance, and documentation of meal intake
- Deliver nutritional supplements to residents at assigned times and assist with intake as needed
- Assist with weighing residents according to facility policy and record weights in designated locations
- Report weight changes to the nurse and supervisor
- Monitor and report any changes in resident skin condition, including pressure areas, skin breakdown, or skin tears
- Assist nursing staff with toileting program activities
- Complete daily flow sheets documenting that assigned tasks were completed and resident progress as directed by supervisor
- Keep the nurse and supervisor informed of any factors that interfere with performing assigned work, such as resident refusal or reassignment to another nursing unit
- Report any changes in a resident's condition to the resident's nurse and supervisor
Required Qualifications
- Current Certified Nursing Assistant certification in good standing with the state
- High school diploma or equivalent
- Current CPR/BLS certification
- Ability to lift, move, and position residents and assist with transfers and other physical activities
- Ability to work effectively as part of a team
- Ability to communicate effectively with residents, families, and staff
