Job Description

Avamere Three Fountains is seeking a Certified Nursing Assistant to provide routine daily nursing care and services to residents in accordance with their individual assessment and care plans. This position is located at 835 Crater Lake Ave. in Medford, Oregon.

Schedule

  • Full-time and part-time positions available
  • Day, evening, and night shifts available

Responsibilities

  • Check residents routinely to ensure personal care needs are being met according to their preferences
  • Review resident care plans daily to identify any changes in daily care routines
  • Inform the nurse supervisor of any changes in resident condition so appropriate information can be entered on the care plan
  • Assist residents with daily needs, including preparation for activities and social programs
  • Transport residents to and from appointments
  • Prepare and maintain residents' rooms to ensure comfort
  • Create and uphold an atmosphere of warmth, patience, and enthusiasm while fostering a calm and cheerful environment
  • Effectively communicate necessary resident information to charge nurses, director of nursing, and administrator
  • Participate in facility surveys by authorized government agencies

Required Qualifications

  • High school diploma or equivalent (10th grade education or above)
  • Current state licensure as a Certified Nursing Assistant
  • Active CPR/BLS certification maintained throughout employment
  • Passion for caregiving and enhancing quality of life for residents
