Job Description
The Center at Blue Hills in Stoughton, Massachusetts is seeking a Certified Nursing Assistant (CNA) to provide direct patient care in our skilled nursing facility. In this role, you will support our interdisciplinary care team in delivering high-quality services to our residents, with a focus on promoting independence and enhancing overall well-being.
Responsibilities
- Provide direct patient care, including assisting with daily living activities, mobility, and hygiene
- Assist with meals, hydration, and nutrition as needed
- Monitor and report changes in patient condition, vital signs, and behavior
- Collaborate with interdisciplinary care teams to develop and implement patient care plans
- Maintain a clean and safe environment, adhering to infection control guidelines
- Document patient care and progress in a timely and accurate manner
Required Qualifications
- Current Certified Nursing Assistant (CNA) certification in the state of Massachusetts
- High school diploma or equivalent
