The Care Team Hospice is seeking a Certified Nursing Assistant (CNA) to provide hands-on personal care and comfort to hospice patients in their homes and care facilities. You will work alongside an interdisciplinary team to help patients maintain dignity, comfort, and quality of life while supporting families through end-of-life care.
Clinical Setting
You will serve patients within approximately one hour of the branch location, covering Lake, Porter, Jasper, and LaPorte counties.
Responsibilities
- Provide personal care including bathing, grooming, hygiene assistance, and comfort measures according to each patient's individualized plan of care
- Observe, document, and promptly report changes in patient condition to the supervising nurse
- Support patients and families with dignity and respect while promoting comfort and quality of life
- Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment
- Complete accurate and timely documentation of all patient care provided in the electronic medical record (Homecare Homebase)
Schedule
- No nights
- No weekends
- No on-call duties
Required Qualifications
- Active Certified Nursing Assistant (CNA) certification
- Previous CNA experience required
- Valid driver's license
- Reliable transportation and current automobile insurance
- Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening

- Hospice, home health, long-term care, or skilled nursing experience
