Certified Nursing Assistant (CNA) - Hospice - PRN
The opportunity
Job Description
We are seeking a dedicated PRN Certified Nursing Assistant to join our hospice care team. In this role, you will provide compassionate personal healthcare and related services to patients in their homes, working under the direction and supervision of a registered nurse. You will assist in creating a safe and clean environment, work cooperatively with patients and families, and share observations and concerns with your supervisor to ensure quality end-of-life care.
Responsibilities
- Provide direct patient care according to the Aide Plan of Care under RN direction
- Assist patients with safe transfers and ambulation per agency policy
- Report changes in patient condition and document care rendered to ensure continuity of care
- Practice infection control principles and maintain a clean, safe, and comfortable environment
- Assist patients with proper nutrition and adequate fluid intake
- Perform treatments and procedures according to agency policy
- Communicate with physicians regarding patient needs and report any changes in condition
- Participate in on-call duties as defined by agency policy
- Ensure equipment and necessary items are available for patient care
- Promote positive, supportive, and respectful communication with patients, families, and staff
- Adhere to all company policies and procedures
- Perform other duties as assigned
Required Qualifications
- High school diploma preferred
- Successful completion of one of the following: A) Training program and competency evaluation as specified in Medicare regulations §418.76(b)(c); B) Competency evaluation program meeting §418.76(c); C) State-approved nurse aide training and competency evaluation program listed in good standing on the state nurse aide registry; D) State licensure program meeting requirements of §418.76(b)(c)
- Current valid driver's license
- Reliable transportation and valid current auto liability insurance
- Ability to read, write, and verbally report clinical information to patients, representatives, caregivers, and agency staff
- Successful completion of the Aide Competency Evaluation Skills Checklist meeting §418.76(c) and written Skills Test
Preferred Qualifications
- At least one year of experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility
- Completion of a Certified Nursing Assistant program
