Certified Nursing Assistant (CNA) Hospice (40797)

The Care Team Hospice is seeking a Certified Nursing Assistant (CNA) to provide hands-on personal care and comfort to hospice patients in their homes and care facilities. This role offers a meaningful opportunity to support patients and families through end-of-life care while maintaining a schedule with no nights, weekends, or on-call duties.

Clinical Setting

You will work within an interdisciplinary hospice care team, serving patients within approximately one hour of the branch location. The role focuses on home-based and facility-based hospice care.

Responsibilities

• Provide personal care including bathing, grooming, hygiene assistance, and comfort measures according to each patient's individualized plan of care
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment
• Observe, document, and promptly report changes in patient condition to the supervising nurse
• Support patients and families with dignity and respect while promoting comfort and quality of life
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase)

Schedule

• No nights, weekends, or on-call duties
• Day shift hours serving patients within approximately one hour of branch

Required Qualifications

• Active Certified Nursing Assistant (CNA) certification
• Previous CNA experience required
• Valid driver's license, reliable transportation, and current automobile insurance
• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening

Preferred Qualifications

• Hospice, home health, long-term care, or skilled nursing experience
