The Care Team Hospice is seeking a Certified Nursing Assistant (CNA) to provide hands-on personal care and comfort to hospice patients in their homes and care facilities. This role offers a meaningful opportunity to support patients and families through end-of-life care while maintaining a schedule that supports work-life balance.
Clinical Setting
You will work as part of an interdisciplinary hospice care team, serving patients within approximately one hour of the branch location.
Responsibilities
- Provide personal care including bathing, grooming, hygiene assistance, and comfort measures according to each patient's individualized plan of care
- Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment
- Observe, document, and promptly report changes in patient condition to the supervising nurse
- Support patients and families with dignity and respect while promoting comfort and quality of life
- Complete accurate and timely documentation of all patient care provided in the electronic medical record (Homecare Homebase)
Required Qualifications
- Active Certified Nursing Assistant (CNA) certification
- Previous CNA experience required
- Valid driver's license, reliable transportation, and current automobile insurance
- Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening
Preferred Qualifications
- Hospice, home health, long-term care, or skilled nursing experience
