Certified Nursing Assistant

We are seeking a Certified Nursing Assistant to provide direct care to residents in a long-term care community. You will support residents' highest level of functioning while maintaining safe, clean environments and promoting quality services in compliance with State and Federal Regulations.

Responsibilities

• Provide personal care to residents according to their individualized plan of care
• Assist residents with activities of daily living including bathing, grooming, dressing, and toileting
• Make incontinent checks at least every 2 hours and provide incontinent care as needed
• Make room checks and verify resident location at least every 2 hours
• Prepare residents for meals and assist with setup, feeding, and transportation to and from dining areas
• Assist with turning, positioning, and repositioning of residents
• Perform and document vital signs, blood pressures, weights, and other trained procedures as directed
• Complete prevention and restorative care including passive range of motion (PROM), active range of motion (AROM), ambulation, transferring, and other ADL care
• Keep residents' rooms neat and clean
• Perform clinical procedures according to community policy
• Document all pertinent information regarding care as assigned
• Receive and communicate shift reports to management on resident status and notify management of any changes in resident medical condition
• Maintain working knowledge of community policies and procedures
• Participate actively in the Interdisciplinary Team Process, including completing assessments and documentation as assigned and attending care plan meetings
• Act as a resident advocate
• Communicate effectively with residents, families, visitors, healthcare team members, and other disciplines
• Participate in Community Quality Assurance and Performance Improvement Programs
• Maintain resident confidentiality and adhere to HIPAA policies
• Maintain current knowledge through ongoing education to comply with State and Federal Regulations

Required Qualifications

• Certification as a Certified Nursing Assistant (CNA) from an approved program
• Successful completion of State CNA course
• Ability to follow facility policies and procedures
• Ability to follow written or verbal instructions
• Basic technical skills to complete repetitive and well-defined duties
• Working knowledge of facility emergency procedures including fire and disaster drills
• Ability to maintain resident confidentiality and adhere to HIPAA policies

Preferred Qualifications

• Six months' experience in long-term care

• Lift constantly 0-10 pounds, frequently 11-50 pounds, and occasionally 51-100 pounds
• Carry constantly 0-10 pounds and frequently 11-50 pounds
• Push constantly 0-10 pounds and frequently 11-50 pounds
• Climb stairs frequently, with opportunity to use elevators as an alternate resource when available
• Bend at the waist, knees, hips, and spine frequently; may require frequently manipulating weights of 25 pounds or more
• Squat frequently to reach below the knee; may require lifting 50 pounds or less
• Twist or rotate occasionally at the trunk, hips, knees, and neck while manipulating weights of 25 pounds or more
• Reach frequently; may occasionally reach overhead requiring manipulation of 10 pounds or less, and below waist requiring manipulation of 25 pounds or more
• Perform work both inside and outside

• Occasional weekend, evening, or night work as needed to ensure shift coverage
