Certified Nurse Aide (CNA) for Home Health- Must Have Own Vehicle

Deer Meadows Home Health is seeking a Certified Nurse Aide (CNA) to provide direct patient care in home health settings throughout Pennsylvania. In this role, you will support patients in their homes by assisting with activities of daily living, monitoring vital signs, and promoting independence and dignity.

Responsibilities

• Provide direct patient care to assigned patients in their homes, including assistance with activities of daily living (ADLs), medication administration, and vital sign monitoring
• Maintain accurate and up-to-date patient records and reports, including vital signs, medication administration, and any notable changes or incidents
• Communicate effectively with patients, families, and healthcare teams to ensure seamless transitions and coordinated care
• Collaborate with other healthcare professionals to develop and implement individualized care plans
• Provide emotional support and companionship to patients, promoting dignity and respect
• Maintain a safe and clean environment for patients, adhering to infection control protocols and policies

Required Qualifications

• Current Certified Nurse Aide (CNA) certification in the state of Pennsylvania
• Must have own reliable transportation; travel reimbursement provided
• Ability to work independently and as part of a team
• Strong communication and interpersonal skills
