Certified Home Health Agency Registered Nurse (CHHA RN)

Excellent Home Care is seeking a Certified Home Health Agency Registered Nurse (CHHA RN) to provide skilled nursing care to adult and pediatric patients in their homes throughout the New York City area. This is a fee-for-service position offering flexible schedules, hours, and patient location choices with excellent work/life balance.

Clinical Setting

You will conduct home visits providing skilled clinical assessments, patient education, and direct nursing care. You will coordinate skilled services within each patient's interdisciplinary team and direct paraprofessional services as needed.

Responsibilities

• Assess patients and families to determine home care needs at the start of care and at regular intervals
• Initiate plans of care and make necessary changes as patient needs evolve
• Evaluate and provide treatment to patients in their homes
• Instruct and advise patients and families in their care
• Administer medications and treatments as prescribed by physicians
• Identify and implement discharge planning needs as part of care development prior to discharge

Required Qualifications

• Valid and current New York State Registered Nurse license and registration
• Current driver's license
• Minimum of 2 years of clinical nursing experience
• Reliable mode of transportation to conduct home visits
• Organized and patient-oriented approach
• Ability to work independently with strong oral and written communication skills
• Competency with Microsoft Office

Preferred Qualifications

• Bachelor of Science in Nursing (BSN) degree
• Prior Certified Home Health Agency (CHHA) experience
• Working knowledge of Home Care Home Base EMR
