Case Manager, Registered Nurse - Field (Williamson, Jackson, Union Counties)

Join CVS Health's Aetna team as a Case Manager, Registered Nurse serving dual eligible populations (Medicare and Medicaid members) in Williamson, Jackson, and Union Counties, Illinois. In this field-based role, you will provide case management services to members with complex health and social challenges, collaborating with members, providers, and community organizations to address their full continuum of healthcare and social determinant needs.

You will work primarily from home with regular field travel to member locations. This position requires independent judgment, critical thinking, and excellent communication skills to facilitate appropriate healthcare outcomes and connect members with available benefits and resources.

Responsibilities

• Utilize critical thinking and clinical judgment to collaborate in the case management process
• Facilitate appropriate healthcare outcomes for members through comprehensive case management
• Assist members with appointment scheduling and benefit access
• Identify and help members access education and care management resources
• Collaborate with members, providers, and community organizations to address health and social determinant needs
• Work independently using virtual collaboration tools and systems

Required Qualifications

• Active and unencumbered Registered Nurse license in the state of Illinois
• Must reside in the state of Illinois
• Minimum 3-5 years of clinical practical experience
• Reliable transportation and willingness to travel 50-75% of the time within Williamson, Jackson, and Union Counties (mileage reimbursed per company policy)
• Ability to work independently and think critically
• Excellent analytical and problem-solving skills
• Effective communication, organizational, and interpersonal skills
• Proficiency with standard corporate software applications including MS Word, Excel, Outlook, and PowerPoint
• Ability to navigate multiple computer systems and applications
• Comfortable setting up and using multiple monitors
• Ability to log into secure systems (VPN, EHR portal), manage strong passwords, and recognize security threats
• Ability to resolve common technical issues independently

Preferred Qualifications

• 2-3 years of care management, discharge planning, and/or home health care coordination experience
• Certified Case Manager credential
• Bilingual capabilities

Education

• Associate's degree required; bachelor's degree preferred

Work From Home Requirements

• Direct/hardwired internet connection to modem/router within 7 feet of computer with minimum 25 Mbps download and 3 Mbps upload speeds (WiFi and satellite internet not permitted)
• Quiet, secure, and private designated home work location free from distractions, compliant with CVS Health and HIPAA guidelines
• Must work within the state and city where you currently reside
• Company provides equipment (keyboard, monitor, computer, headset); you must provide workspace furniture (desk or standing desk)
• Commitment to obtaining required internet speeds and adhering to all work-from-home requirements

Schedule

• Full-time position
• 40 anticipated weekly hours

Compensation and Benefits

• Base salary range: $66,575.00 - $142,576.00 annually (actual offer depends on experience, education, geography, and other relevant factors)
• Eligible for CVS Health bonus, commission, or short-term incentive program in addition to base pay
