Join Aetna's Long-Term Services and Supports (LTSS) team as a field-based Case Manager Registered Nurse serving members throughout the Roanoke, Virginia area and surrounding regions. In this role, you will assess, plan, and coordinate comprehensive care for members with diverse medical and behavioral health needs. Through frequent in-person visits (up to 75% travel), you will build strong relationships, gain insights into members' environments, and develop tailored care plans that promote both immediate health improvements and long-term wellness.
You will integrate services, address barriers to care, and connect members with appropriate resources across healthcare and community systems. Working closely with interdisciplinary teams, providers, and support networks, you will ensure care is proactive, coordinated, and aligned with each member's unique goals.
Responsibilities
- Conduct in-depth, face-to-face assessments to identify member needs
- Develop and implement individualized care plans
- Coordinate medical, behavioral, and community-based services
- Monitor progress and adjust care strategies to improve outcomes
- Advocate for members to ensure access to high-quality, appropriate care
Required Qualifications
- Current unrestricted Virginia state RN licensure
- Minimum 3 years of clinical experience, preferably in medical-surgical nursing, behavioral health, or long-term care
- Minimum 1 year of managed care experience
- Reside in Roanoke or surrounding regions in Virginia
- Reliable transportation and willingness to travel up to 75% of the time; mileage is reimbursed per company policy
- Proficiency with computer skills, including navigating multiple systems and keyboarding
Preferred Qualifications
- 5 years of clinical practice experience
- Case management experience in an integrated model
- Effective verbal and written communication skills
- Bilingual in English and Spanish
- Long-term services and supports (LTSS) experience
Education
- Associate's Degree or Bachelor's Degree in Nursing with active RN licensure required; Bachelor's Degree in Nursing preferred
Schedule
- Full-time position, 40 hours per week
- Monday through Friday, 8:00 AM to 5:00 PM
- Work-from-home flexibility with structured weekday hours
Compensation and Benefits
- Base salary range: $60,522.00 to $129,615.00 annually
