CareBridge Advance Practice Provider, Nurse Practitioner

CareBridge Health, part of the Elevance Health family, is seeking a Nurse Practitioner to provide primary and urgent healthcare to patients receiving home and community-based services through state Medicaid programs, dual eligible members, and other managed care populations. This virtual position offers full-time remote work with required in-person training sessions, providing flexibility while ensuring essential onboarding and skill development.

You will collaborate with company physicians, patients' other healthcare providers, and family members to develop complex plans of care aligned with patients' health status, goals, and values. You will work with adult patients facing chronic and complex conditions, including co-morbid medical and mental health diagnoses, limited personal resources, and multiple chronic medical conditions.

Responsibilities

• Provide primary and urgent healthcare via telephone and telehealth video to assigned patients
• Develop and implement clinical plans of care for adult patients with chronic and complex conditions
• Gather history, perform physical exams, order diagnostics as needed, and develop treatment plans based on patient goals and current conditions
• Identify and close gaps in care
• Meet patients' and families' physical and psychosocial needs with support from the interdisciplinary team
• Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence, and crisis anticipation and prevention
• Maintain contact with clinical team members, patients' other physicians, and other medical providers to coordinate optimal care and resources
• Maintain patient medical records and documentation consistent with state regulations and company standards
• Prescribe medication as permitted by state prescribing authority
• Participate in continuing education as required by state and certifying body

Required Qualifications

• Master's degree in Nursing
• Active, national NP certification
• Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Texas, Arizona, Kansas, or Iowa
• Valid, current, active RN Compact license
• Two or more years of experience managing complex care cases
• Experience with Electronic Medical Records (EMR)

Preferred Qualifications

• DEA registration or eligibility
• Experience managing complex care cases for patients with developmental disabilities and chronic illnesses

Schedule and Location

• Monday through Friday, 8:00 AM to 5:00 PM CST
• Rotating on-call duties
• Virtual work with required in-person training sessions
• Licensed in Texas, Arizona, Kansas, or Iowa; candidates must reside within reasonable commuting distance from an office location unless accommodation is granted as required by law
