Cardiology Nurse Practitioner (NP)

KOPA, Inc. is seeking a Cardiology Nurse Practitioner to provide clinical care and leadership at Avante at Jacksonville, a skilled nursing facility serving a specialized patient population with complex infection control needs. As an advanced practice registered nurse, you will diagnose and treat health conditions, conduct physical examinations, interpret diagnostic tests, and oversee patients' overall health while promoting disease prevention and healthy practices.

This role includes both direct patient care and supervisory responsibilities, requiring you to work collaboratively with other healthcare professionals and lead a team of nurse practitioners.

Responsibilities

• Examine patients and review medical records
• Order and interpret diagnostic tests including blood work and imaging
• Diagnose health conditions and illnesses
• Propose and implement treatments for chronic and infectious diseases
• Prescribe medications
• Maintain accurate patient records and schedules
• Inform patients about their health conditions and prescribed medications
• Educate patients and family members on diseases, epidemics, and prevention strategies
• Oversee patients' overall health and promote good health practices
• Advance professional knowledge and conduct research
• Supervise and mentor nurse practitioners on the team
• Review documentation and provide clinical feedback
• Lead team meetings
• Assist with onboarding and training of new nurse practitioners
• Monitor performance and support ongoing professional development
• Act as a liaison between the nurse practitioner team and facility leadership
• Assist in creating and refining workflows, protocols, and best practices
• Provide coverage at other Florida facilities as needed, which may involve occasional in-state travel

Required Qualifications

• Valid nurse practitioner license
• Proven experience as a Nurse Practitioner in a skilled nursing facility
• Minimum one year of supervisory experience
• Expertise in primary care, acute care, and chronic care practices
• Knowledge of health and patient care regulations
• Excellent communication skills
• Strong ethics
• Patience and composure under pressure
• Willingness to continue gaining knowledge and clinical experience
