Bilingual Licensed Practical Nurse LPN Ave Maria

Join our Home Health team as a Bilingual Licensed Practical Nurse in an office-based, full-time position. In this role similar to an Intake Coordinator, you will manage patient records, coordinate admissions, and ensure all required documentation is complete, accurate, and compliant. Your clinical knowledge will be essential in supporting and streamlining internal processes.

Responsibilities

• Manage and maintain patient charts and records in electronic and/or physical formats
• Ensure all required documentation is complete prior to patient admission
• Track and follow up on missing documentation
• Assist in organizing and maintaining compliance-ready patient files
• Support the clinical team with administrative tasks related to patient care coordination

Required Qualifications

• Active Licensed Practical Nurse (LPN) license in Florida
• Bilingual proficiency in Spanish and English
• Strong organizational, multitasking, and communication skills
• Ability to work in a fast-paced office environment

Preferred Qualifications

• Previous experience in Home Health or intake coordination
• Experience with EMR systems
