BHMP Nurse Practitioner (NP Full Time Day)

Connections Health Solutions is seeking a Nurse Practitioner to provide and coordinate comprehensive medical and psychiatric care for individuals receiving crisis stabilization services. This physician-led, data-driven organization specializes in immediate-access behavioral health crisis care, combining medical and recovery-oriented treatment to stabilize individuals in crisis and connect them to community-based resources.

Clinical Setting

You will work within a behavioral health crisis care facility, serving individuals of various ages (children, adolescents, adults, and families) who are experiencing psychiatric crises. The role is part of an interdisciplinary team model focused on crisis intervention, stabilization, and safe discharge planning.

Responsibilities

• Serve as the attending clinician for assigned individuals and coordinate comprehensive, individualized care throughout their stay
• Complete initial psychiatric evaluations, focused diagnostic assessments, crisis and family interventions, risk assessments, and assessments of capacity to consent to treatment
• Incorporate medical, psychological, social, behavioral, and developmental aspects into treatment and discharge planning
• Collaborate with individuals, families, and interdisciplinary team members to establish treatment goals, evaluate progress, update treatment plans, and provide education
• Coordinate safe and effective discharge planning to promote continuity of care
• Document timely and accurate clinical information in the health record, including admission orders, progress notes, treatment decisions, and discharge orders
• Ensure medication education and informed consent are completed in accordance with established policies
• Review nursing health assessments, obtain relevant medical histories, and conduct focused physical examinations as clinically indicated
• Order and review laboratory and diagnostic tests in accordance with clinical standards and use findings to guide treatment decisions
• Communicate and coordinate with primary care physicians, emergency department clinicians, outpatient behavioral health providers, psychiatric providers, and other community partners
• Provide clinical guidance to the interdisciplinary team regarding individual needs, treatment approaches, medication management, safety risks, level-of-care decisions, and treatment-plan updates
• Apply evidence-based practices when prescribing and managing medications
• Collaborate with internal and external partners to evaluate individuals receiving voluntary or involuntary services, review petitions, complete required assessments, and coordinate with the legal system as applicable
• Participate actively in interdisciplinary treatment-team meetings, medical staff meetings, regional and quarterly meetings, and other meetings requested by Medical Leadership
• Promote a respectful, collaborative, and team-based care environment through effective communication and shared decision-making

Required Qualifications

• Active and unrestricted Pennsylvania Registered Nurse license
• Current authorization to practice as a Certified Registered Nurse Practitioner (CRNP/APRN) in Pennsylvania
• Completion of an accredited Nurse Practitioner program with master's level or higher preparation
• Current Psychiatric-Mental Health Nurse Practitioner (PMHNP) certification, if required for assigned scope of practice
• Current DEA registration with authority to prescribe controlled substances, including Schedule III medications, as applicable to the role
• BLS (Basic Life Support) certification must be obtained within seven days of hire
• Pass Pennsylvania background checks: Child Abuse Clearance, FBI Criminal Background Check, and Pennsylvania State Police Criminal Record Check
• Pass medical screenings including physical examination, TB test, and drug test
• Full vaccination against COVID-19 with proper documentation, or otherwise be exempt pursuant to the Company's exemption process, prior to start date
• Complete all credentialing requirements within the required time period, including obtaining a National Provider Identifier (NPI), Medallion enrollment, Medicare enrollment, and any other necessary credentials as required for the role

Preferred Qualifications

• Dual certification as a Family Nurse Practitioner (FNP) or other relevant advanced practice specialty
• Doctorate of Nursing Practice (DNP) degree
• Experience providing psychiatric, behavioral health, crisis stabilization, addiction treatment, or recovery services
• Experience providing care to children, adolescents, adults, and/or families in a behavioral health or crisis setting

Schedule and Employment

• Full-time position, day shift
• Eligible for $10,000 commitment bonus for full-time employment
