Job Description

Lifepoint Health's Access Point Behavioral Health Transfer Center is seeking a Licensed Practical Nurse (LPN) to join a remote team coordinating patient placements across 18 locations. Operating 24/7, the center processes over 6,000 transfers monthly, serving as a centralized hub for assessing clinical needs, verifying bed availability, and arranging transportation for individuals requiring acute behavioral health care.

This remote position offers the opportunity to work in a dynamic, fast-paced environment alongside 31 experienced mental health professionals, including clinicians, nurses, EMTs, and case managers.

Responsibilities

  • Screen and coordinate patient admissions and transfers from hospitals and clinics
  • Use clinical decision-making and approved criteria to ensure appropriate level of care placement
  • Collaborate with intake teams, physicians, nursing staff, and hospital departments to facilitate timely placements
  • Monitor patient flow, track delays, and help improve access processes
  • Serve as a liaison between facilities, patients, families, and providers
  • Navigate referral submissions from initial contact to final acceptance within 45 minutes
  • Ensure compliance with EMTALA, Lifepoint Health policies, and quality standards
  • Demonstrate strong clinical judgment and critical thinking in a high-volume setting
  • Type 30-40 words per minute
  • Successfully complete Internet Speed and Basic Computer Competency Assessments

Required Qualifications

  • Active Compact LPN License
  • 3 or more years of clinical experience in Behavioral Health, Emergency Department (EmPath Unit), or Critical Care with Behavioral Health patients
  • High-speed internet access (minimum 50 Mbps upload / 100 Mbps download)
  • Excellent communication skills and commitment to patient-centered care
  • Knowledge of regulatory requirements, EMTALA, Lifepoint Health Ethics and Compliance policies, and quality initiatives

Schedule

  • Full-time, varied shifts
  • 7 p.m. to 7 a.m. or 7 a.m. to 7 p.m.

  • Salary range: $51,611 minimum, $64,514 midpoint, $77,417 maximum
