APP - Certified Nurse Practitioner | Hospice

GSS IA Ottumwa Hospice seeks a Certified Nurse Practitioner to provide holistic, compassionate care to patients and families across a large rural service area in Iowa. Working in collaboration with physicians, staff RNs, and an interdisciplinary hospice team, you will diagnose, manage, and prevent acute and chronic illness while promoting wellness and quality of life for hospice patients.

Clinical Setting

The hospice agency is based in Ottumwa, Iowa, and provides services throughout Appanoose, Davis, Jefferson, Keokuk, Mahaska, Marion, Monroe, Van Buren, Warren, and Wapello counties. You will work with an established interdisciplinary team supported by 24/7 on-call nursing coverage.

Responsibilities

• Perform in-depth health risk assessments on all assigned clients
• Review and interpret medical, surgical, and social history
• Perform appropriate screening procedures and interpret laboratory and diagnostic data
• Reconcile medications and recommend appropriate treatments or follow-up actions
• Prescribe or perform various treatment modalities including medications, injections, wellness counseling, suturing, and ancillary testing
• Evaluate clients with acute changes in condition and collaborate with the primary care provider to implement appropriate interventions
• Provide medical expertise on pain, symptom management, and evaluation of terminal prognosis to clients, families, and staff
• Collaborate with the interdisciplinary team to create, review, and revise the client's plan of care
• Assist in meeting regulatory requirements related to timely client certification for services
• Identify unmet needs and gaps in care, and initiate appropriate referrals
• Maintain compliance with OSHA and State Board of Nursing requirements
• Maintain control of all protected health information (PHI) in accordance with HIPAA
• Participate in mandatory compliance and training programs
• Perform other duties as assigned

Schedule

• Monday through Friday, 8:00 a.m. to 4:30 p.m.
• 8-hour day shifts
• PRN (as-needed) employment with varying weekly hours

Required Qualifications

• Master's, postmaster's, or doctorate degree from a nurse practitioner program accredited by the Commission on the Collegiate of Nursing Education or National League for Nursing Accrediting Commission
• Minimum two years of clinical experience as a Nurse Practitioner in geriatric, adult, or family practice setting
• Demonstrated current competence and provision of care, treatment, or services for an adequate volume of patients in the past twelve months, or completion of master's/postmaster's degree program in the past twelve months
• Current licensure by the applicable state board of nursing for advanced practice
• Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances
• Current certification by the American Nurses Credentialing Center or an equivalent nationally recognized body
• Current Scope of Practice approved by the applicable state Board of Nursing for advanced practice
• Obtain and maintain required department-specific competencies and certifications

Preferred Qualifications

• Experience with palliative care or hospice
• For candidates licensed prior to August 1, 1995 in North Dakota: master's degree in nursing is preferred
