Advanced Practice Nurse, APN, Maine West High, School Based Clinic, Des Plaines - Part Time

This part-time Advanced Practice Nurse position is based at Maine West High School's school-based clinic in Des Plaines, serving a pediatric and adolescent population in a primary care setting.

You will provide comprehensive clinical care including assessment, diagnosis, and management of acute and chronic conditions appropriate to the school-based health center environment. Your responsibilities will include ordering and interpreting diagnostic studies, prescribing pharmacologic and non-pharmacologic interventions, and providing health promotion, disease prevention, and disease management education to students and families.

Responsibilities

• Assess, diagnose, and determine or alter treatment and management plans based on clinical indication, evidence-based care, cost effectiveness, and risk/benefit analysis
• Order, perform, and interpret appropriate diagnostic studies
• Prescribe pharmacologic and non-pharmacologic interventions and therapies
• Provide health promotion, disease prevention, and disease management counseling and education
• Manage patients as part of an interdisciplinary team within scope of practice
• Seek physician or other healthcare team member consultation and referrals as appropriate
• Escalate need for more emergent or specialized care when necessary
• Maintain accurate, complete, concise, and timely documentation in the electronic medical record
• Substantiate and submit professional services consistent with compliant coding and billing practices
• Facilitate consistent, coordinated care and clear communication among healthcare team members and community agencies
• Perform office or hospital procedures in accordance with specialty practice, competency, and granted privileges
• Participate in quality, safety, and peer review initiatives, organizational and departmental meetings, and committees
• Participate in education and onboarding of new team members, students, and other healthcare professionals
• Maintain standards of productivity, access, face-to-face time, and quality metrics
• Demonstrate knowledge and skills necessary to provide age-appropriate care and understand principles of growth and development across the lifespan

Required Qualifications

• Active RN license in state(s) of practice
• Active APRN, APRN-FPA, APNP, or other APRN license in state(s) of practice
• Active national board certification in area of clinical practice and populations served
• Active DEA registration prior to hire
• Active Illinois Controlled Substance License prior to hire (if practicing in Illinois)
• Active BLS and/or ACLS, PALS, NRP as required by clinical practice prior to or within 6 months of hire
• Master's Degree in Nursing
