Liberty Medical is seeking an Adult Nurse Practitioner to join our outpatient medical practice in Holmdel, NJ, providing comprehensive, patient-centered care with a focus on geriatric medicine, chronic disease management, and preventive care. This position is ideal for a provider who enjoys building meaningful relationships with older adults in an outpatient office setting.
Clinical Setting
You will work in a collaborative outpatient office environment alongside physicians and the healthcare team, with an emphasis on personalized treatment and long-term patient relationships.
Responsibilities
- Perform comprehensive medical evaluations and physical examinations
- Diagnose and treat acute and chronic medical conditions
- Provide ongoing management of common geriatric conditions and multiple chronic illnesses
- Develop individualized treatment plans based on each patient's medical needs and goals
- Manage and monitor medications, including medication reconciliation and appropriate adjustments
- Provide preventive healthcare, screenings, and health education
- Assess functional status, fall risk, cognitive concerns, and other age-related issues
- Coordinate care with physicians, specialists, family members, and other healthcare professionals
- Order and interpret laboratory tests, diagnostic studies, and other appropriate evaluations
- Educate patients and families regarding diagnoses, treatment plans, medications, and lifestyle modifications
- Maintain accurate and timely electronic medical records
- Participate in quality-improvement initiatives and contribute to a collaborative practice environment
Required Qualifications
- Current APRN/Nurse Practitioner license in the State of New Jersey
- Board certification as a Nurse Practitioner through an accredited national certifying organization
- DEA registration and appropriate prescribing privileges
- Excellent clinical assessment, communication, and interpersonal skills
- Ability to work independently while collaborating effectively with physicians and other members of the healthcare team
- Strong organizational skills and attention to detail
Preferred Qualifications
- Geriatric, Adult-Gerontology Primary Care, Family Nurse Practitioner, or closely related experience
- Experience caring for older adults and patients with multiple chronic conditions
- Background in geriatric medicine, primary care, internal medicine, skilled nursing, long-term care, assisted living, or other settings serving older adults
Schedule
- Monday through Friday: 9 AM to 5 PM
- No Wednesday hours
- No weekend shifts
- 32 hours per week
