Pine Park Health is seeking an experienced, independently licensed Nurse Practitioner to provide primary care services to older adults living in senior living communities across the East Bay San Francisco area (Oakland, Walnut Creek, Concord). This role is designed for a seasoned NP ready to practice at the top of their license, autonomously managing a complex patient panel within a value-based primary care model without the constraints of a traditional clinic setting.
Clinical Setting
Pine Park Health brings healthcare directly to seniors' homes within senior living communities, offering prevention, screening, chronic condition management, lab work, and diagnostic testing. You will conduct weekly community visits and work collaboratively with facility staff to address comprehensive health needs. The clinical team includes dedicated RNs and Care Coordinators who handle operational responsibilities, allowing you to focus on patient care.
Responsibilities
- Provide onsite primary care services to older adults living in senior living communities
- Develop and execute detailed person-centered care plans for all primary care patients
- Provide monthly Chronic Care Management (CCM) visits and care; review and sign off on CCM documentation
- Work with the Medical Director to identify and develop policies addressing issues that affect day-to-day practice, culture, or clinical guidelines, including for emerging diseases
- Run regular huddles and provide leadership to the clinical team
- Identify ways of working with senior living management and care teams to better carry out Pine Park care plans and improve patient outcomes
- Practice independently and manage a complex patient panel without direct physician oversight
Required Qualifications
- Active California NP-103 Independent Practice License
- Minimum 5 years of Nurse Practitioner experience
- Demonstrated ability to practice independently and manage a complex patient panel without direct physician oversight
- Valid driver's license and willingness to travel within the East Bay San Francisco area
- Comfortable with tech-enabled care delivery tools used to coordinate patient care and team function
Preferred Qualifications

Compensation and Benefits
Base salary range: $177,000–$187,000 total target (includes base and performance bonus), equity, and benefits. Final compensation reflects experience, depth of clinical background, licensure, and geographic market factors.
