Glens Falls Center for Rehabilitation and Nursing, a 120-bed rehabilitation and skilled nursing facility located on the border of Warren and Washington Counties, is hiring an RN Admissions/Wound Nurse in Glens Falls, NY.
This role focuses on initiating admission assessments for new residents and hospital returns, developing initial care plans, and coordinating the admission process with the interdisciplinary team. You will review Physician's Resource Information (PRI) and participate in discharge planning when necessary. The position includes responsibility for auditing records to ensure compliance with facility standards and evaluating residents' care plans when clinical conditions warrant changes.
Responsibilities
- Initiate admission assessments on new residents and hospital returns
- Develop and implement initial care plans
- Assist with the development and direction of the Nursing Department as directed by the Director of Nursing
- Review PRI and coordinate all components of the admission process with the interdisciplinary team
- Audit records to ensure compliance with facility standards
- Participate in discharge planning when necessary
- Review and evaluate residents' care plans when conditions warrant changes
- Perform required charting according to policy and procedure
- Complete MDS assessments, care plans, and other documents as assigned
- Maintain confidentiality of all resident information in accordance with HIPAA and corporate guidelines
- May supervise or direct resident care as necessary
Required Qualifications
- Active and unrestricted New York State Registered Nurse License
- Prior experience as an admissions nurse in long-term care settings
- Excellent clinical assessment skills
- Ability to make sound independent decisions
- Strong leadership and supervisory skills
- Proficiency with Microsoft Office (Excel, Word, Outlook)
- Excellent communication and interpersonal skills
